Pelvic Pain: Could These 3 Factors Be Driving Your Symptoms?

Chronic pelvic pain can have many different drivers, from digestive issues, inflammation, endometriosis, pelvic floor dysfunction, and hormonal imbalances. Nutrition can be a powerful place to start, particularly when digestive function, inflammation, or hormone dysregulation are contributing to your symptoms.

Let’s look at three interconnected areas that commonly influence symptoms: gut motility, inflammation, and hormones.

 

#1. Gut Health & the Role of Gut Motility

The gut and pelvic region share a small amount of real estate within the abdomen, meaning constipation, bloating, and intestinal distention can create additional pressure in an already sensitive pelvic region. 

In our clinic we’ve seen a connection between poor gut motility, gut dysbiosis, and chronic pelvic pain. For example, one client experiencing significant digestive symptoms and pelvic pain saw substantial symptom resolution after identifying and addressing underlying small intestinal bacterial overgrowth (SIBO).

Gut motility refers to the muscular contractions that move food and waste through the digestive tract. When motility slows, constipation can develop and food remains in the gut longer. This increases microbial fermentation and can contribute to gut dysbiosis, fungal or bacterial overgrowth, as well as SIBO.

One important regulator of motility is the migrating motor complex (MMC), a series of muscular contractions that helps move residual food and bacteria through the upper digestive tract between meals. Leaving about 4-5 hours between meals rather than continuously grazing allows the MMC to complete these “cleansing” cycles.

Fiber is typically the go-to for supporting regular bowel movements, but more isn’t always better if you struggle with pelvic pain. A balance of soluble and insoluble fiber supports stool formation, elimination, and a healthy gut microbiome. Foods such as chia and flax seeds, beans, and lentils provide a mixture of fiber types and are especially supportive for gut motility.

Prebiotic and fermentable fibers feed beneficial bacteria. However, highly fermentable fibers may worsen gas, bloating, and pelvic pressure when bacterial overgrowth is present. Rather than eliminating these foods indefinitely, the goal should be to address the underlying dysbiosis and gradually rebuild fiber tolerance.

Additional motility support includes adequate hydration, mineral and potassium intake, regular movement, and eating prokinetic foods before meals, which naturally stimulate gastrointestinal movement, such as ginger and artichoke.

 

#2. Factors that Fuel Inflammation & How to Keep It in Check

Inflammation is another important piece of the pelvic pain puzzle. Endometriosis is a common cause of chronic pelvic pain and is characterized by endometrial-like tissue growing outside of the uterus, with inflammation and immune dysregulation playing important roles in disease progression and pain. By addressing factors that contribute to excessive inflammatory signaling while incorporating anti-inflammatory nutrition strategies, we can help create an environment that may reduce pelvic pain.

The gut microbiome and intestinal barrier also play important roles in regulating immune activity. Disruptions to either can contribute to inflammatory signaling beyond the gut, creating an important connection between gut health, immune regulation, and conditions associated with chronic pelvic pain.

One group of key anti-inflammatory nutrients is omega-3 essential fatty acids, which serve as precursors to specialized compounds involved in resolving inflammation. Fatty fish such as salmon, sardines, trout, and mackerel provide the omega-3s EPA and DHA, while chia seeds, flax seeds, hemp seeds, and walnuts provide the plant-based omega-3 ALA.

A diverse intake of colorful plant foods can provide additional anti-inflammatory support through antioxidants and polyphenols – think dark berries, leafy greens, herbs, spices, extra virgin olive oil, and green tea.

We can also support a healthier inflammatory balance by minimizing refined sugars and highly processed or hydrogenated oils, particularly when these foods are used instead of more nutrient-dense options.

Nutrition can help provide the building blocks needed to regulate and resolve inflammation, but ultimately we have to identify and address the other underlying drivers of chronic immune activation. These may include gut dysbiosis, viral infections, toxic burden, mold mycotoxins, and more. The goal isn’t to eliminate inflammation entirely, since it’s a necessary body process, but to give the body the nutritional resources it needs to keep inflammatory responses appropriately regulated.

 

#3. How Hormones Influence Pelvic Pain

Hormones are another important consideration, particularly when pelvic pain fluctuates throughout the menstrual cycle or is associated with an estrogen-related condition such as endometriosis.

Estrogen can stimulate the growth and activity of endometrial tissue, which makes both overall estrogen levels and healthy estrogen metabolism essential areas to consider. A high estrogen environment can speed up tissue growth and worsen pelvic pain. This can be from overtly high estrogen levels, or normal estrogen levels that are not counterbalanced by progesterone if progesterone levels are low. It’s important to understand your individual hormone patterns before taking steps to “balance” your hormones.

Next, it’s essential to support the body’s natural ability to appropriately detoxify or break down, utilize, and eliminate hormones such as estrogen. This brings us back to the gut. After estrogen is broken down by the liver, a portion is eliminated through the digestive tract. Regular bowel movements and adequate fiber help facilitate this elimination, while the makeup and activity of the gut microbiome may also influence estrogen metabolism and recirculation. Supporting a healthy gut in turn supports a healthy hormone environment.

Phytoestrogen-rich foods such as freshly ground flax seeds can also be valuable. Despite their name, phytoestrogens don’t simply “increase estrogen.” These plant compounds interact with estrogen receptors differently than our own estrogen and may help support balanced estrogen signaling, regardless of whether estrogen levels are high or low.

Finally, don’t overlook thyroid health. Thyroid hormones, particularly active Free T3, interact with reproductive hormones while also influencing gut motility and menstrual regularity. Adequate energy and protein intake, alongside minerals such as selenium, zinc, iodine, iron, and magnesium, help provide the nutritional foundation for healthy thyroid hormone production and metabolism.

 

Addressing the Drivers, Not Just the Symptoms

Gut health, inflammation, and hormones all continuously influence one another. Poor gut motility can alter the health of the gut microbiome and estrogen elimination. Gut dysfunction can influence inflammatory signaling. Hormones can affect both reproductive tissues and digestive function, and are influenced by chronic inflammation.

Nutrition may not be the entire answer to chronic pelvic pain, but supporting these three foundational areas can help address factors that may be significantly contributing to symptoms. Persistent or severe pelvic pain should always be appropriately evaluated so that conditions such as endometriosis, pelvic floor dysfunction, gastrointestinal disorders, and other potential causes aren’t overlooked.
