The Presenting Picture

This case was brought forward by a clinician seeking mentorship after feeling clinically “stuck” with a complex presentation involving thyroid autoimmunity, mineral dysregulation, immune instability, and impaired iron metabolism.

The patient presented with:

GI dysfunction: Low commensal bacteria, high staph overgrowth, low pancreatic elastase
Hematologic + iron markers: Low ferritin, low serum iron, high MCV
Thyroid dysfunction: Borderline low thyroid hormones, elevated reverse T3, high thyroid antibodies
Immune markers: Low WBC
Metabolic markers: Low ALP, low cholesterol
Mineral imbalances:
Low copper, ceruloplasmin
Low manganese, cobalt
Low sodium, potassium
Elevated zinc

Clinically, she was also experiencing:

Fatigue and low resilience
Infrequent bowel movements (as low as 2x/week)
Recent miscarriage (5–6 weeks gestation)
Post-miscarriage hormonal disruption 
Increased thyroid antibodies following initiation of detox protocols

The Outcome

What I Identified + Guided the Clinician On

The primary issue wasn’t gut dysfunction, thyroid imbalance, or iron deficiency in isolation—it was a system that lacked the energy and resources to respond appropriately to stress or intervention.

I helped the clinician recognize that:

Thyroid dysfunction was largely adaptive and downstream, driven by low energy availability and nutrient insufficiency
The immune system was not overactive, but reactive due to being under-resourced

Most importantly, I guided her to shift away from pushing detoxification and antimicrobial strategies, and instead focus on:

Building foundational capacity first (digestion, minerals, energy production)
