Why Your Thyroid Medication Isn’t Working & How to Make it Work Better
To treat underactive thyroid, or hypothyroidism, thyroid medication is often the first step. While thyroid hormone replacement can be incredibly helpful, it shouldn’t be the only treatment strategy. Maybe you’ve been told to take your thyroid medication on an empty stomach and sent on your way. Months later, your thyroid labs improve marginally, yet you continue struggling with fatigue, brain fog, constipation, weight changes, or infertility.
The reason for this disconnect between lab values and symptoms is that thyroid medication replaces thyroid hormones, but it doesn’t restore the body’s ability to produce, convert, transport, and use those hormones efficiently. In this guide, we’ll explain why supporting your thyroid physiology matters just as much as taking medication correctly, along with simple nutrition and supplement strategies to help you get the most from your treatment.
Why Your Thyroid Medication Isn’t Working
Thyroid hormone replacement medications increase the amount of thyroid hormone circulating in your bloodstream. But having more hormones available doesn’t necessarily mean your body can use them efficiently.
Research estimates that up to 15% of people continue experiencing hypothyroid symptoms despite thyroid lab values falling within the reference range once on thyroid hormone medication.
For thyroid hormones to have their intended effects, they must first be absorbed through the digestive tract, transported through the bloodstream, converted into their active form (T3), bind to thyroid receptors, and ultimately support energy production inside your mitochondria. Each of these steps depends on healthy physiology.
At Functional Fueling, we refer to the underlying factors that disrupt these processes as Energy Leaks. These include: nutrient deficiencies, immune activation, exercise stress, and toxic burden. These Energy Leaks place additional demands on the body while reducing its ability to efficiently produce cellular energy (ATP). These same factors also can impair thyroid hormone production, conversion, and reduce cellular sensitivity to thyroid hormones.
This helps explain why simply taking thyroid medication and increasing thyroid hormones doesn’t always eliminate symptoms. If the underlying energy leaks haven’t been addressed, you may have more thyroid hormone circulating in your bloodstream, improved laboratory values, and yet very little change in how your body actually functions.
Successfully treating an underactive thyroid requires more than simply replacing thyroid hormone in order to normalize thyroid labs (namely TSH, thyroid stimulating hormone). It means restoring the physiology that allows your body to properly produce, convert, transport, and utilize thyroid hormones. Sometimes that means medication works more effectively, sometimes it reduces the amount of medication needed over time, and for others it helps them feel significantly better while remaining on the same dosage of medication.
How to Maximize Thyroid Medication Absorption and Effectiveness
While addressing the Energy Leaks influencing thyroid physiology, there are also key strategies to ensure you’re absorbing as much of your medication as possible to effectively treat your underactive thyroid.
Thyroid hormone is absorbed best on an empty stomach. Food slows gastric emptying and can bind to thyroid hormone, reducing the amount that is actually absorbed and enters your bloodstream. Even small reductions in absorption can significantly influence the effects of medication and contribute to fluctuating TSH and thyroid hormone levels over time. Even if thyroid labs normalize, inconsistent absorption can result in ongoing hypothyroid symptoms.
For most people, the best rules of thumb are to:
- Take your thyroid medication with water immediately upon waking.
- Wait at least 30 to 60 minutes before eating breakfast or drinking coffee.
- Most importantly, be consistent. Taking your medication the same way every day helps maintain more stable thyroid hormone levels. If mornings aren’t a reliable time for you to take thyroid medication, take it consistently at bedtime at least 3 to 4 hours after your last meal.
Foods and Supplements that Interfere with Thyroid Medication
Several foods and supplements can reduce thyroid hormone absorption when taken too close to your medication. Keep these interactions in mind when taking your medication and planning your supplement routine.
Nutrition Considerations
- Coffee is one of the biggest culprits and may significantly reduce thyroid medication absorption. Be sure to wait at least 30 to 60 minutes before your morning coffee, ideally waiting until after breakfast before having caffeine to support your nervous system.
- Soy foods, including soy milk, tofu, and edamame, may also decrease absorption. Soy is also a goitrogenic food, which can interfere with iodine uptake by the thyroid, impairing thyroid hormone production. It’s worth assessing overall soy intake as well as the timing of it in relation to thyroid medication.
- Cruciferous vegetables like broccoli, kale, Brussels sprouts, and cauliflower also naturally contain goitrogens. While they do not directly interfere with thyroid medication absorption, individuals with thyroid dysfunction should enjoy them cooked, and avoid excessive amounts.
- Oxalate-rich foods such as spinach, almonds, and beets are similar to cruciferous vegetables in that they don’t directly impair thyroid medication absorption. However, when consumed in large amounts can bind many of the beneficial minerals required for thyroid hormone production and conversion.
- High-fiber foods or fiber supplements, such as psyllium husk, oat bran, bran cereals, can bind thyroid hormone within the digestive tract and slow medication absorption. Rather than avoiding fiber altogether, prioritize fiber in the meals farthest from thyroid medication.

Supplement Considerations
- Calcium and iron are among the most well-established inhibitors of thyroid hormone absorption and should be taken at least four hours away from thyroid medication. This applies to prenatal multivitamins, mineral complexes, and calcium carbonate supplements (also known as Tums).
- Betaine hydrochloride (HCl) has somewhat of the opposite effect. Instead of binding thyroid hormones, it actually increases stomach acid production, therefore causing much more thyroid medication to be absorbed. This is an important consideration to ensure that supplementation and medication is being appropriately monitored in order to avoid pushing thyroid hormones too high and developing symptoms of overactive thyroid activity, such as heart palpitations and anxiety.
- Herbs such as bugleweed and lemon balm have traditionally been used to support individuals with hyperthyroidism by potentially slowing thyroid activity. While they aren’t fully contraindicated for those on thyroid hormone medications, they should be used with caution and under the guidance of a knowledgeable practitioner.
The Bottom Line: Treat Underactive Thyroid from All Angles
Thyroid medication can be an incredibly valuable tool, but it’s only one aspect of restoring healthy thyroid function, and may not have to be a lifelong resource. Replacing thyroid hormone doesn’t automatically correct the underlying physiology needed to produce energy, convert hormones efficiently, or respond to those hormones at the cellular level.
Addressing the Energy Leaks contributing to thyroid dysfunction allows the medication you’re taking to work with your body, not simply around it. It also may open the door to decreasing or fully weaning off of thyroid hormone medication if appropriate.
