FFN Chronic Fatigue Case Studies: Two Clients Transformed by Addressing the Body’s Hidden Energy Leaks

Chronic fatigue can be one of the most frustrating symptoms to navigate because there are often multiple underlying factors contributing to low energy, brain fog, poor exercise performance, slow recovery, and other fatigue-related challenges. Rather than being the root problem itself, chronic fatigue is often a signal that deeper imbalances are impacting the body’s ability to produce and sustain energy.

Patient 1: Samantha

Samantha is a 27-year-old female who came to Functional Fueling to get to the root of her chronic fatigue, painful acne, poor workout recovery, anxiety, brain fog, and PCOS/PMOS-like symptoms. 

Clinical Picture:

  • Suspected PCOS/PMOS, with known elevated DHEA-S and DHT (the most potent testosterone metabolite) 
  • Hormonal IUD for the past 11 years

Symptoms:

  • Severe fatigue and brain fog
  • Poor sleep (frequent waking overnight, not waking feeling rested)
  • Anxiety and heart palpitations
  • Painful, systemic acne (chest, back, shoulders, glutes, neck, and arms)
  • Abnormal dark course hair growth
  • Poor workout recovery (soreness lasting 2-3 days, needing extended recovery within workouts, hit a plateau and unable to progress training load)
  • Difficulty gaining weight and building muscle

 

The Initial Clinical Picture: Fatigue, Hormonal Imbalances, and Immune Dysfunction

Samantha had worked with other functional medicine practitioners before, but was still seeking answers and resolution. Amidst completing graduate school and working high-stress jobs, the struggles of her recent health journey had left her feeling like she had lost her sense of self and intuition.

She was struggling with extreme fatigue that was impacting nearly every aspect of her life. She experienced significant afternoon crashes, including episodes of dozing off while driving, alongside debilitating brain fog that was affecting her work performance. 

She was also struggling with workout recovery and prolonged soreness following exercise. Despite wanting to build strength and muscle, she felt stuck, unable to progress her workouts and gain muscle.

At the same time, she was dealing with widespread acne across her chest, back, shoulders, glutes, neck, and arms, along with dark coarse hair growth and PMS irritability. She struggled to get restful sleep, waking multiple times throughout the night, while anxiety and heart palpitations were an ongoing concern.

Comprehensive testing quickly revealed several physiological “energy leaks” driving her fatigue and other symptoms. 

 

Samantha’s Energy Leak 1: Nutrient Depletion

She was depleted of critical essential nutrients like B vitamins, vitamin A, iodine, and omega-3 fats. This stemmed from low energy availability and insufficient micronutrient replenishment, impacting mitochondrial function and hormone production. Her cholesterol levels were also too low to adequately support hormone production, while low leptin confirmed low energy availability and impaired hormone signaling.

 

Samantha’s Energy Leak 2: Energy Stress

Samantha’s energy stress was largely driven by inadequate fueling around exercise, impaired oxidative stress balance, as well as overtraining. Her oxidative stress balance was significantly impacted by the iron overload discovered through lab testing. Excess iron in the body is a key driver of oxidative stress, inflammation, and immune stress. 

Elevated evening cortisol aligned with her restless sleep and afternoon energy crashes, and furthered her energy stress picture. 

 

Samantha’s Energy Leak 3: Toxic Burden

Finally, mineral testing revealed elevated mercury and aluminum levels. Heavy metal buildup in the body increases the toxic burden on the liver, further exacerbating the strain on Samantha’s detoxification systems and mitochondrial function.

All of these factors contributed to a much deeper pattern of metabolic, hormonal, adrenal, and inflammatory dysfunction.

Samantha’s Review:

 

Patient 2: Liliana

Liliana is a 28-year-old female who came to Functional Fueling desperate for answers and resolution of her debilitating chronic fatigue and brain fog, coupled with severe PMS and PMDD (premenstrual dysphoric disorder) symptoms, poor sleep, and sugar cravings.

Clinical Picture:

  • Suspected PMDD (premenstrual dysphoric disorder) 
  • Known history of mold exposure
  • History of anemia

Symptoms:

  • Severe chronic fatigue and brain fog (difficulty waking up in the morning, afternoon crash, evening crash)
  • Severe PMS breast tenderness, mood fluctuations, and cramping
  • Anxiety and decreased stress resilience
  • Excessive hair shedding
  • Constipation, bloating, poor appetite, and sluggish digestion
  • Uncontrollable sugar cravings
  • Difficulty losing weight
  • Facial hair growth
  • Poor sleep (frequent waking in the early morning 2-4am)

 

The Initial Clinical Picture: Fatigue, Hormonal Imbalances, and Immune Dysfunction

Liliana’s symptoms began about two years prior following a traumatic personal life event alongside transitioning into a new, significantly higher-stress job. What initially started as fatigue evolved into debilitating exhaustion and brain fog that she described as “ruining her life.” 

The fatigue felt deep within her body, making it difficult to wake up in the morning and causing multiple crashes throughout the day. She would often fall asleep on the couch before dinner, disrupting her sleep cycle and contributing to blood sugar imbalances that frequently caused her to wake up between 2-4am.

As her chronic fatigue worsened, her stress resilience continued to decline. Her anxiety became more severe and was often amplified by hormonal fluctuations throughout her menstrual cycle, leading her to suspect PMDD. She was also experiencing significant PMS-related breast tenderness and cramping leading up to her period. 

Due to the severity of her fatigue and brain fog, Liliana initially overlooked the significance of her digestive symptoms until we explored them more deeply. She was dealing with chronic constipation, relying on caffeine to have a daily bowel movement, alongside bloating, sluggish digestion, poor appetite, and intense sugar cravings that felt difficult to control. 

Her chronic fatigue, brain fog, and anxiety also made it difficult to maintain healthy routines and sustainable lifestyle changes, which further intensified her struggles with weight loss resistance and feeling uncomfortable in her body.

 

Liliana’s Energy Leak 1: Toxic Burden

Comprehensive testing revealed that Liliana’s symptoms were being driven by a significant toxic burden that was impairing detoxification, hormone metabolism, and mitochondrial function. She was aware of past mold exposure, and mycotoxin testing confirmed a significant mycotoxin burden contributing to her fatigue, brain fog, anxiety, and hormone-related symptoms. 

Given her intense sugar cravings, bloating, sluggish digestion, and constipation, there was also strong suspicion of gut dysbiosis and possible fungal overgrowth. Mold, fungal, and yeast overgrowth, often fuel one another, increasing toxic load while further impairing gut integrity and detox capacity.

Testing also showed impaired phases 1 and 2 liver detoxification, contributing to poor estrogen and androgen metabolism, further driving inflammation, hormonal symptoms, and compounding her toxic burden.

 

Liliana’s Energy Leak 2: Immune Activation

Liliana’s elevated mycotoxin burden was also contributing to significant immune activation, another major energy leak. Chronic immune activation can create a constant “fight-or-flight” nervous system state, draining energy reserves and worsening fatigue, anxiety, brain fog, and hormonal imbalances. This aligned with her elevated cortisol awakening response (CAR) seen on her DUTCH test.

The combination of mycotoxins, immune activation, and oxidative stress played a major role in her chronic fatigue, disrupted sleep, poor stress resilience, impaired mitochondrial energy production, and hormone regulation.
